NEWSLETTER
Publication of all disciplinary actions by the Board is required by law and is
available on a quarterly basis, whether there is a printing of the
newsletter or not. The information contained in this report was accurate
at the time of printing. However, for the most up-to-date
information, interested persons should contact the Board office. Additionally,
the description of the licensee's conduct is general in
nature and anyone interested in a complete description of the licensee's conduct
should request that documentation from the Board
office.
Victoria A. Roth, Speech Language Pathologist, St. Charles, Missouri
Cause of Discipline: 345.065.2( 11) RSMo The Board found cause to discipline
license
based upon Licensee's failure to obtain thirty (30) hours of acceptable
continuing education during the period
January 1, 1999 through December 31, 2000. Board Action: License is placed on
Probation for
a period of two (2) years. Licensee must, within six (6) months of the effective
date of this agreement, docu-ment
attendance and completion of four (4) hours of continuing education. These hours
must be in addition
to the number of CE hours required for renewal of li-cense.

The Board found cause to discipline license based upon a finding of the
Administrative Hearing
Commission that Licensee's conduct constituted mis-conduct, fraud, unethical
conduct and unprofessional
conduct in the performance of the duties of his profes-sion.
Board Action: License is Publicly Reprimanded. License is also restricted in
that Licensee shall not per-form
any spinal surgery and any major surgery, includ-ing, but not limited to, hip
and knee replacements.

The Board found cause to discipline license based upon Licensee's withdrawal of
application for
reappointment of privileges and his failure to appeal hospital's denial of
reappointment of his privileges which
resulted in a final disciplinary action by the hospital against licensee.

Rosemary Rodgers, M. D., Fulton, Illinois Cause of Discipline:

The Board found cause to discipline license based upon final disciplinary action
taken against Licen-see
by the Illinois Department of Professional Regula-tion when it suspended
Respondent's license to practice
medicine in the State of Illinois. Board Action: License is Suspended for three
(3) years or until the Board receives a "fit to practice" letter from Licensee's
treating physician.

and (8),
RSMo 2000 The Board found cause to discipline license
based upon action taken against Licensee by the Iowa Board of Medical Examiners.
The Iowa Board placed
license on probation for a period of five (5) years due to Licensee's
unprofessional conduct.
Board Action: License is placed on Probation for a period of five (5) years to
run concurrent with the Iowa
Order. Effective Date: 05/ 30/ 2003
Russell L. Bradley, M. D., Lenoir, North Carolina Cause of Discipline:

The Board found cause to discipline license based upon Licensee writing a
prescription for hydroco-done
for an individual who was not a patient or for other than medically accepted
therapeutic purposes or not in
the course of professional practice. Board Action: License is Publicly
Reprimanded.

The Board found cause to discipline license based upon issuance of a public
reprimand, fine, audit
and requirement for additional CME's by the Georgia State Board of Medical
Examiners due to Licensee's
failure to conform to minimal standard of acceptable medical practice in the
care of a patient.
Board Action: License is Publicly Reprimanded. Licensee also required to
complete, and provide proof of
additional thirty (30) hours of CME in pediatrics. Effective Date: 06/ 16/ 2003
Theodore Roberts, M. D., Sikeston, Missouri Cause of Discipline:
334.100.2 (4) (h), (5), (6),
and (10), RSMo The Board found cause to discipline based
upon Licensee's failure to properly supervise a physi-cian assistant employed by
Licensee. Licensee failed to
review physician assistant's work, records and practice activity which allowed
physician assistant to practice
medicine outside the scope of her profession as a physi-cian assistant.

(4) (h), and (5),
RSMo The Board found cause to discipline license
based upon Licensee's misconduct and misrepresenta-tion, prescribing controlled
substances without authority
to do so, practice which is or might be harmful or dan-gerous to the physical or
mental health of a patient, and
violation of state drug laws. Board Action: License is Publicly Reprimanded.

The Board found cause to discipline license based upon Licensee's failure to
submit documentation verifying that he obtained the required thirty (30) hours
of continuing education for the period January 1, 2000 through December 31,
2001.
Board Action: License is Publicly Reprimanded. Licensee also must complete and
submit proof of ob-taining
thirty (30) hours of continuing education within six (6) months of the effective
date of this order.

The Board found cause to discipline license based upon Respondent's conviction
of two (2) felony
counts of Conspiracy to Defraud the United States Gov-ernment.
Board Action: License is Revoked. Respondent may not apply for reinstatement of
license for a period of

The Board found cause to discipline license based upon Licensee's entry of
guilty pleas to felony
counts of fraudulently obtaining/ attempting to obtain hy-drocodone in the
counties of Camden, Cole and Miller,
Missouri. Board Action: License is Revoked.

The Board found cause to discipline license based upon Respondent's entry of a
guilty plea to six (6)
felony counts of Health Care Fraud. Board Action: License is Revoked.

Donald W. Kreutzer, M. D., Clarksville, Missouri Cause of Discipline:
334. 100. 2( 4)( h), (5) and
(10), RSMo Supp. 1994 The Administrative Hearing Commission found
cause to discipline license based upon Licensee's pre-scribing of Oxytocin for a
patient without examining the
patient. In addition, Licensee assisted and enabled a person to engage in the
practice of midwifery when that
person was not registered or currently eligible to prac-tice.
Board Action: License is Suspended for sixty (60) days, to be followed by a
period of Probation for
three (3) years to begin immediately following the Sus-pension. Licensee must
complete a medical ethics
course and a prescribing course during the first six (6) months of probation.

The Board found cause to discipline license based upon Licensee's conviction on
fourteen (14) fel-ony
counts of Delivery of a Controlled Substance and one (1) felony count of Public
Aid Vendor Fraud in the
State of Illinois. Board Action: License is Revoked. Licensee
shall not make application for reinstatement of license for a period of five (5)
years.

2000 The Board found cause to discipline license
based upon Licensee's failure to keep psychiatric ap-pointments and failure to
participate in MAOPS treat-ment
program, in violation of the Settlement Agreement of December 26, 2001.
Board Action: License placed on Probation for an additional seven (7) years.

the medication for himself. Board Action: License is placed on Probation for

2000 The Board and Licensee entered a Joint Stipu-lation
of Facts, Waiver of Hearings Before the Adminis-trative Hearing Commission and
State Board of Regis-tration
for the Healing Arts, and Consent Order with Joint Proposed Findings of Fact and
Conclusions of Law
(Joint Stipulation). The Joint Stipulation established cause to discipline based
upon Licensee's guilty pleas
to charges of violation of order of protection, harass-ment by telephone and
obstructing a peace officer.
Board Action: License is Suspended for a pe-riod of three (3) years or until an
acceptable "fit to prac-tice"
letter is provided to the Board. Period of Suspen-sion is to be followed by
Probation for a period of five (5)

suffered manic episodes

The Board found cause to discipline license based upon Licensee entering into a
Settlement Agree-ment
with the New York State Board of Professional Medical Conduct whereby an order
of final disciplinary
action in the form of a fine and probation, was issued by the New York Board.
Board Action: License is Retired in lieu of Disci-plinary action.

(g), (6) & (8) RSMo The Board found cause to issue a probated li-cense.
Licensee was convicted of thirteen (13) felony counts of Mail Fraud and Aiding
and Abetting and
Money Laundering and Aiding and Abetting. The Mis-souri Board had previously
revoked license.
Board Action: License is issued on Probation for a period of five (5) years in
lieu of Denial of licensure.

Kapeun Chung, M. D., Farmington, Missouri Cause of Discipline: 334.100.2(
25) RSMo
The Board found cause to discipline license based upon Licensee's failure to
obtain a passing score
on the Special Purpose Examination (SPEX) and failure to be re-certified by the
American Board of Surgery. Li-censee
was allowed two (2) attempts on both exams, within six (6) months of the date of
the order.
Board Action: License is Retired in lieu of Disci-pline.

medical practice, and negligence. Board Action: License is Voluntarily

Michael R. Simmons, M. D., Girard, Kansas Cause of Discipline: 334.

(5) & (8) RSMo The Board found cause to probate license
based upon disciplinary action taken against license by the Kansas Board of
Healing Arts. The Kansas Board
suspended Dr. Simmons' medical license for a period of thirty (30) days and,
thereafter, indefinitely restricted his
license due to his engaging in sexual relationships while maintaining
physician-patient relationships.
Board Action: License to practice medicine is-sued on Probation for a period of
five (5) years.

The Board found cause to discipline license based upon a Settlement Agreement
