The Board found cause to discipline license based upon Licensee entering into a BNDD Agreement which placed Licensee’s Missouri Controlled Substance registration on two (2) years probation.
Board Action: License placed on Probation for a period of two (2) years, retroactive to October 20, 2003. Licensee shall attend and successfully complete a prescribing course and a record keeping course, approved by the Board, within one (1) year of the effective date of this order.

The Board found cause to discipline license based upon Licensee entering into a Settlement Agreement with BNDD wherein BNDD found that Licensee had submitted application to prescribe controlled substances that contained false information. In addition, Licensee issued prescriptions in Missouri on a terminated BNDD registration and failed to comply with BNDD record keeping requirements.
Board Action: License is placed on Probation for a period of two (2) years, retroactive to March 24, 2003, the effective date of the BNDD Settlement Agreement. Licensee shall comply with all terms of the BNDD Settlement Agreement and the Board Settlement Agreement.
