DISCIPLINARY ACTIONS SINCE FALL 2002

Publication of all disciplinary actions by the Board is required by law and is available on a quarterly basis, whether there is a printing of the newsletter
or not. The information contained in this report was accurate at the time of printing. However, for the most up-to-date information, interested persons
should contact the Board office. Additionally, the description of the licensee's conduct is general in nature and anyone interested in a complete
description of the licensee's conduct should request that documentation from the Board office.
 

Mary Meehan, SLP, St. Louis, Missouri Cause of Discipline: §345.065. 2( 3), (6) and
(11), RSMo The Board found cause to discipline license
based upon Licensee's failure to obtain the required thirty (30) hours of acceptable continuing education
during the period January, 1, 1999 through December 31, 2000. Licensee incorrectly indicated on her appli-cation
that she had met the continuing education re-quirements.
Board Action: License is placed on Probation for a period of two (2) years with an additional require-ment
that Licensee obtain ten (10) hours of CE within (6) months of the date of this agreement.

Regina (Mayfield) Winkler, SLP, St. Louis, Missouri Cause of Discipline: §345.065. 2( 3), (6) and
(11), RSMo The Board found cause to discipline license
based upon Licensee's failure to obtain the required thirty (30) hours of acceptable continuing education
during the period January 1, 1999 through December 31, 2000. Licensee incorrectly indicated on her appli-cation
that she had met the continuing education re-quirements.
Board Action: License is placed on Probation for a period of two (2) years with an additional require-ment

The Board found cause to discipline license based upon Licensee's failure to obtain the required
seventy-five (75) hours of acceptable CME during the period January 1, 1998 through December 31, 2000.
Board Action: License is Publicly Repri-manded. Licensee must obtain eleven (11) hours of
AMA Category I CME within six (6) months of the ef-fective date of this agreement and must provide docu-mentation
to the Board within thirty (30) days of com-pletion.

Kathryn Schott, SLP, Fenton, Missouri Cause of Discipline: §345.065.2( 5), RSMo
Board Action: The Board found cause to disci-pline license based upon Licensee's failure to obtain
the required thirty (30) hours of continuing education as required for renewal of licensee.
Board Action: License is placed on Probation for a period of two (2) years. Licensee must also ob-tain
thirty (30) CE hours in addition to the hours re-quired for license renewal.

Robert J. Oliver, D. O., St. Charles, Missouri Cause of Discipline: §334.100. 2( 4) and (5),
RSMo The Board disciplined license based upon the
Administrative Hearing Commission's finding of unpro-fessional conduct and conduct or practice that is or
might be harmful or dangerous to the mental or physi-cal health of a patient. Licensee failed to be present
during patient's post-operative care and failed to dele-gate such care to another physician.
Board Action: License is Publicly Repri-manded.

Abdul Moheet, M. D., Chesterfield, Missouri Cause of Discipline: §334.100.2( 5), RSMo
The Board disciplined license based upon the Administrative Hearing Commission's finding of gross
negligence and conduct that might have been harmful or dangerous to the physical health of a patient in that
he diagnosed, treated and discharged a patient with-out knowing the patient's blood pressure or medical
history. Board Action: License is Publicly Repri-manded.
 

Effective Date: 11-19-02. Board's Order ap-pealed to Cole County Circuit Court on December 19,
2002.
Peggy Taylor, D. O., St. Louis, Missouri Cause of Discipline: §334.100.2( 23), RSMo
 

The Board found cause to discipline license based upon Licensee's violation of the terms of a Set-tlement
Agreement entered into with the Bureau of Narcotics and Dangerous Drugs effective January 17,
2001. Licensee failed to meet the recordkeeping, in-ventory and dispensing requirements of the Agree-ment.
 

Board Action: License is placed on Probation for a period of five years.

Mary Riebold, P. T. A., Barnhart, Missouri Cause of Discipline: §334.100.2( 6), RSMo
The Board found cause to discipline license based upon Licensee's continued practice as a physi-cal
therapist assistant after the January 31, 2001, ex-piration of her license.
Board Action: License is Publicly Repri-manded.

Bret A. Hubbard, D. O., Jenks, Oklahoma Cause of Discipline: §334. 100.2( 1), (13) and
(22), RSMo The Board found cause to discipline license
based upon Licensee's diversion and consumption of controlled substances, in violation of his Probationary
agreement with the Board. Board Action: License Suspended for three (3)
years or until such time as a "fit to practice" letter is provided by the Missouri Physicians Health Program
and Licensee's treating physician. Period of Suspen-sion to be followed by a Probationary period of ten (10)
years. Effective Date: 12-11-02
 

Kapeun Chung, M. D., Fredericktown, Missouri Cause of Discipline: §334.100. 2( 4)( g), (5) and
(6), RSMo The Board found cause to discipline license
based upon Licensee's clinical privileges having been Suspended due to Licensee's failure to have a nurse
chaperone present during physical examinations, and his failure to perform appropriate medical assess-ments
of two (2) patients. Board Action: License is Publicly Repri-manded.

The Board found cause to discipline license based upon final disciplinary action taken by the Ari-zona
Board of Medical Examiners due to Licensee's unprofessional conduct and negligence involving his
failure to provide emergency/ timely treatment to a pa-tient.

Brad J. Angelos, M. D., New Madrid, Missouri Cause of Discipline: §334.100.2( 5), RSMo
Supp. 1987 Court of Appeals reversed the judgment of Cir-cuit
Court of Scott County and Supreme Court denied Appellant's Application to Transfer case to the Su-preme
Court. Therefore, Board's Revocation Order of August 11, 1997, is affirmed. Court's Stay Order is
dissolved. Board Action: License is Revoked.

Kelly L. Johnson, M. D., Joplin, Missouri Cause of Discipline: §334.100.2( 4)( g), RSMo
The Board found cause to discipline license based upon termination of Licensee's employment
with St. John's Regional Medical Center after Licensee was found to be using propofol without a valid pre-scription.
 

Board Action: License is Publicly Reprimanded and placed on Probation for a period of five (5) years.

The Board found cause to discipline license due to Licensee's failure to maintain her license in a
current status while practicing as a speech-language pathologist.

The Board and Respondent entered into a Joint Stipulation whereby Respondent admits to
fraudulently obtaining controlled substances for his own use.
Board Action: License is Suspended for three (3) months followed by a period of five (5) years Pro-bation.

The Board found cause to discipline license based upon Licensee's guilty plea to one (1) felony

Michael K. Thornton, M. D., St. Peters, Missouri Cause of Discipline: §334.100.2( 4), (4)( i) and
(5), RSMo The Administrative Hearing Commission is-sued
its Decision finding cause to discipline license based upon Licensee's inappropriate sexual com-ments
and conduct with regard to patients which con-stitutes misconduct, unethical conduct, unprofessional
conduct, conduct or practice that is or might be harm-ful or dangerous to the physical or mental health of the
patients. Board Action: License is Revoked with no ap-plication
for reinstatement for a period of seven (7) years.

Miles James Jones, M. D., Lee's Summit, Missouri Cause of Discipline: §334.103, RSMo
The Board found cause to discipline license based upon the final and unconditional Revocation of
license by the North Dakota State Board of Medical Examiners for writing prescriptions over the Internet
without first seeing the patients. Board Action: License is Revoked with no ap-plication
for reinstatement for a period of two (2) years and one (1) day.

Michael Liston, M. D., Independence, Missouri Cause of Discipline: §334.103, RSMo
The Board found cause to discipline license based upon Licensee's guilty plea to one (1) class C
felony count of Assault in the Second Degree. Board Action: License is Revoked.

Michael S. Otto, P. T. A., Alhambra, Illinois Cause of Discipline: §334.100.2( 6), RSMo
The Board found cause to discipline license based upon Licensee's failure to submit the documen-tation
of the thirty (30) hours of continuing education required to renew his physical therapist assistant li-cense.
 

Board Action: License is Publicly Reprimanded and Licensee must obtain thirty (30) hours of continu-ing
education and submit proof of attendance and completion within six (6) months of this Agreement.

RSMo Supp. 1994-1999 The Board found cause to discipline license
based upon Licensee's attempt to obtain a fee for

Cause of Discipline: §345.065.2( 11), RSMo The Board found cause to discipline license
based upon Licensee's failure to obtain the required category of continuing education hours for the report-ing
period January 1, 1999 through December 31, 2000.
Board Action: License placed on Probation for a period of two (2) years.

Judith A. Nations, M. D., St. Louis, Missouri Cause of Discipline: §334.100.2( 6), RSMo
The Board found cause to discipline license based upon Licensee's failure to provide documenta-tion
of the required number of CME hours for the year 1999.
Board Action: License is Publicly Repri-manded. Licensee is required to complete twenty-five
(25) additional hours of Category I CME in addition to the fifty (50) hours required for renewal by December
31, 2003. Effective Date: 3-7-03
 

David M. Sherman, M. D., Columbia, Missouri Cause of Discipline: §334.100. 2( 1) and (2),
RSMo The Board found cause to discipline license
based upon Licensee's use of alcohol to the extent that such use impaired his ability to perform the work
of a physician. In addition, Licensee entered a plea of guilty to driving while intoxicated, an offense involving
moral turpitude. License is placed on Probation for a period of
five (5) years. Effective Date: 3-7-03
 

Sherry Hutchins, D. O., Roach, Missouri Cause of Discipline: §334.100.2( 1), (4)( h), (13)
an (22), RSMo 1994, Supp. 1997 and 2000. The Board found cause to discipline license
based upon Licensee's misappropriation of controlled substances for her own use by writing false prescrip-tions,
bearing fictitious names and addresses. Board Action: License is Suspended for a pe-riod
of three (3) years or until Board receives a "fit to practice" letter from treating physician. Period of Sus-pension
to be followed by a period of Probation for seven (7) years to begin immediately following Sus-pension.

Terry L. Ray, D. O., Columbia, Missouri Cause of Discipline: §334.100. 2( 1), (4)( g) and
(22), RSMo The Board found cause to discipline license
based upon Licensee's abuse of the narcotic Demerol to the extent he was unable to perform the duties of a
physician, resulting in his privileges at University Hos-pital being restricted.
Board Action: License placed on Probation for a period of seven (7) years.

Lanny M. Turner, M. D., Chesterfield, Missouri Cause of Discipline: §334.100.2( 1), RSMo
The Board found cause to discipline license based upon Licensee having been treated for sub-stance
abuse after previously having his licensed on Probation for substance abuse.
Board Action: License is placed on Probation for a period of seven (7) years.

Steven Cicero, D. O., North Kansas City, Missouri Cause of Discipline: §334.103 RSMo
The Board found cause to discipline license based upon Licensee entering a plea of guilty to one
(1) felony count of Unlawful Sale of Prescription Drug Samples.
Board Action: License is Revoked. Effective Date: 04/ 13/ 2003
 

Patrick M. Coyne, M. D., St. Louis, Missouri Cause of Discipline: §334.103 RSMo
The Board found cause to discipline license based upon Licensee's having been found guilty of
Unlawful Use of a Weapon -Carrying a Concealed Weapon, a Class D felony.
Board Action: License is Revoked. Effective Date: 04/ 14/ 2003
 

Michael D. Landreneau, M. D., Joplin, Missouri Cause of Discipline: §334.103, RSMo
The Board found cause to discipline license based upon Licensee having been adjudicated guilty
of one (1) felony count of Endangering the Welfare of a Child.
Board Action: License is Revoked with no ap-plication for reinstatement for a period of seven years.

Kenneth G. Petty, M. D., Chico, California Cause of Discipline: §334.103, RSMo
The Board found cause to discipline license based upon Licensee's plea of guilty in the State of
Utah to a felony charge of Possession of a Controlled Substance, to wit: psilocybin mushrooms.
Board Action: License is Revoked. Effective Date: 04/ 15/ 2003

Timothy M. Meehan, D. O., Springfield, Missouri Cause of Discipline: §334.100. 2( 4)( h), (5) and
(13) RSMo The Board found cause to discipline license
based upon Licensees prescribing of hydrocodone in amounts which were excessive and medically unac-ceptable,
thereby endangering the health of patients A and B. In addition, Licensee failed to properly docu-ment
controlled drugs prescribed to patients A and B. Board Action: License is Publicly Repri-manded.
Licensee must complete both a prescribing course and a record-keeping course within six (6)
months of the effective date of this agreement. Effective Date: 04/ 15/ 2003
 

Charles M. Counce, D. O., Fulton, Missouri Cause of Discipline: §334.100. 2( 4)( h) and (5),
RSMo The Board found cause to discipline license
based upon Settlement Agreement entered wherein Licensee acknowledged that he had over-prescribed
Ultram to a patient in 2000 and 2001, to the extent that it was considered excessive and improper and endan-gered
the patients health. Board Action: License is Publicly Repri-manded.
Licensee must also attend and successfully complete a prescribing course and a record-keeping
course within six (6) months of the effective date of this order. Licensee must submit documentation of
completion of courses within thirty (30) days of com-pleting each course.

William H. Whitley, D. O., Orangeburg, South Caro-lina
Cause of Discipline: §334.100.2 (8), RSMo The Board found cause to discipline based
upon action taken by the South Carolina Board of Medical Examiners. The South Carolina Board issued
a final order which imposed a fine and publicly repri-manded License.
Board Action: License is Publicly Repri-manded. Licensee must attend and successfully com-plete
a prescribing course approved by the Board and must notify the Board within thirty (30) days of comple-tion
of the course. Effective Date: 04/ 22/ 2003
 

Constantino Carpio, M. D., Troy, Missouri Cause of Discipline: §334.100.2( 4), (4)( h) &
(13) RSMo 1994 The Board and Licensee entered a Joint Stipu-lation
in the case pending at the Administrative Hear-ing Commission in which Licensee admitted to violat-ing
the drug laws of the state of Missouri. Board Action: License placed on probation for
a period of five (5) years; Licensee must surrender BNDD and DEA registrations and shall not hold a
 

BNDD or DEA registration during period of probation. Effective Date: 04/ 28/ 2003
M. C. Walton, M. D., Mountain View, Missouri Cause of Discipline: §334.100.2( 4)( o), RSMo.
The Board found cause to discipline license based upon Licensee's violation of the Settlement
Agreement entered into with the Board effective July 21, 2000. Licensee wrote prescriptions for controlled
drugs but failed to document the drugs in patient's charts.
Board Action: License is placed on probation for a period of ten (10) years. Licensee must also sur-render
BNDD and DEA authority to prescribe con-trolled substances.
Effective Date: 04/ 28/ 2003
Harry M. Katz, M. D., Cedar Hill, Missouri Cause of Discipline: §334.100.2( 2) RSMo
The Board found cause to discipline license based upon Administrative Hearing Commission Deci-sion
which found that Licensee's conduct or practice is or might have been harmful or dangerous to the physi-cal
or mental health of the patient and that Licensee's conduct was negligent.
Board Action: License placed on probation for a period of five (5) years. Licensee shall immediately
surrender BNDD and DEA prescribing authority. Licen-see must also attend and complete a prescribing
course within six (6) months of the date of this Order. Effective Date: 04/ 28/ 2003
Boards Order currently on appeal in Circuit Court of Jefferson County, Missouri.
 

MISCELLANEOUS ACTIONS
Reno Cova, M. D., Cape Girardeau, Missouri
Pursuant to request by Licensee, the Board ap-proved
amending existing Order to permit Licensee to apply for BNDD and DEA Schedule II prescribing
authority. In addition, upon request of Licensee, the restriction which prohibits Licensee from performing
neurosurgery was removed. Board Action: Amended Order
Effective Date: 1-28-03
Ralph Cooper, D. O., Joplin, Missouri Judgment of Circuit Court of Cole County af-firmed
Board's Findings of Fact, Conclusions of Law and Disciplinary Order of May 1, 2002, which revoked
license based upon Licensee's failure to obtain a passing score on the SPEX exam after four (4) at-tempts.
 

Board Action: License Revocation reinstated by Judgment of the Cole County Circuit Court.
Effective Date: 2-5-03

Brad Angelos, M. D., New Madrid, Missouri The Board determined that Licensee had met
all requirements for reinstatement, specifically, he re-ceived a passing score on SPEX exam, submitted an
evaluation from Professional Renewal Center and completed required CME hours.
Board Action: Licensee issued an unrestricted license.
Effective Date: 3-10-03
Manuel Largaespada, M. D., Eureka, Missouri The Medical Licensing Board of Indiana Re-voked
license to practice in Indiana. Missouri Board took action based on Revocation in Indiana. Revoca-tion
of Indiana license was set aside. Board Action: Missouri Board Order Revoking
license rescinded. License reinstated. Effective Date: 3-26-03
 

Richard H. Sundermann, M. D., Columbia, Missouri Licensee requested that the Board remove its
Restriction of his license due to the fact he has com-pleted his sixty (60) days of pro bono psychiatric serv-ices
as required. Board Action: Amended Order to remove re-quirement
that Licensee's practice limited to Family Health Center in Columbia, Missouri.
Effective Date: 3-27-03
David R. Stone, M. D., Springfield, Missouri Licensee requested that the Board approve
Kenneth Dugan, M. D., as his practice monitor. Board Action: Amended Order to allow Ken-neth
Dugan, M. D., to serve as Licensee's practice monitor.
Effective Date: 3-27-03
Jennifer Sloan, P. T., Cameron, Missouri Cause of Discipline: §334.100.2( 1), RSMo
The Board found cause to discipline license based upon Licensee's failure to participate in Mis-souri
Physicians Health Program or Missouri Associa-tion of Osteopathic Physicians and Surgeons program,
in violation of Probation agreement entered into effec-tive October 23, 2001.
Board Action: License is retired in lieu of disci-pline.

James Nanson, M. D., Columbia, Missouri Cause of Discipline: §334.100.2( 4)( g), RSMo
The Board found cause to discipline license based upon the fact that Licensee's privileges were
summarily Suspended by Columbia Regional Hospital due to his use and abuse of oxycodone. Licensee
failed to appeal the Suspension. Therefore, the Sus-pension became permanent disciplinary action.
 

Board Action: License is retired in lieu of Disci-pline.

The Board found cause to discipline license based upon Licensee's failure to obtain the required
