The Board found cause to discipline license based upon action taken against Licensee’s Ohio license by the State Medical Board of Ohio. Licen-see’s Ohio medical license was Reprimanded and Permanently Restricts Licensee from prescribing, administering, or personally furnishing controlled substances, anorectic medications or diuretics for weight loss. Licensee also admitted to engaging in sexual conduct with two (2) patients while they were under his care.
Board Action: License is Suspended for a period of eighteen (18) months effective November 11, 2003. Suspension to be followed by Probation for a period of five (5) years.

The Board found cause to discipline license based upon Revocation of Licensee’s Missouri Controlled Substances Registration. BNDD revoked Licensee’s Controlled Substances Registration after Licensee authorized numerous prescriptions for Schedule II controlled substances, although he only had authority to prescribe schedules III, IV and V controlled substances.
Board Action: License is Suspended for a period of sixty (60) days, effective December 14, 2004. Suspension shall be immediately followed by Probation for a period of ten (10) years from the date of the termination of the period of Suspension.

The Board found cause to discipline license based upon Licensee’s failure to enroll in a structured individualized education plan as required by CPEP, failed to timely notify the Board of his changes of address and failed to comply with monitoring visits by Board’s representatives. The Settlement Agreement rescinds Board’s Findings of Fact, Conclusion of Law and Disciplinary Order issued March 4, 2004.
Board Action: License is Revoked with no reapplication for a period of two (2) years and one (1) day. However, should Licensee be accepted into a residency program, and provided he meets all other licensure eligibility requirements, Revocation shall be stayed and Licensee will be issued a temporary license to participate in the residency training program.

The Board found cause to discipline license based upon Licensee’s submission of operative reports of seven (7) different patients stating that he performed surgeries when he was not actually the hands-on surgeon. The Board found cause to discipline license based upon Licensee's failure to perform physical examinations and obtain medical histories prior to prescribing controlled drugs. In addition, Licensee failed to maintain appropriate patient records regarding controlled drugs, including correct names and addresses for prescriptions and properly labeled containers. Licensee prescribed controlled drugs in amounts which exceeded recommended dosages. Licensee's conduct constitutes repeated negligence, incompetency or practice which might be harmful or dangerous to the mental or physical health of a patient.
Board Action: License is Voluntarily Surrendered in lieu of Discipline.

Pursuant to Settlement Agreement dated September 18, 2002, license was placed on five (5) years probation and Licensee was required to surrender his BNDD and DEA Registrations. The Board found cause to discipline license based upon the fact that on or about June 28, 2001, Licensee’s privileges at St. Luke’s Hospital in Kansas City, Missouri (St. Luke’s) were Summarily Suspended for disruptive behavior, namely Licensee was alleged to have used rude, insulting or demeaning language. On or about September 4, 2001, St. Luke’s withdrew the Suspension of Licensee’s privileges in exchange for Licensee’s resignation of his privileges at St. Luke’s.
Board Action: The Board accepted Voluntary Retirement of License in lieu of Discipline.
